We are offering an exciting opportunity for a Clinical Pharmacist to take on a newly created role, working for both Solent NHS Community & Mental Health Trust and Southampton City (North) Primary Care Network.
Reporting to the Lead Network Clinical Pharmacist your time will be divided in a 50/50 split between Southampton City (North) PCN and the Solent Pharmacy Team in Southampton.
Split of Duties:
For Southampton City (North) PCN you will assist with medicines optimisation and quality improvement initiatives as required and utilise clinical knowledge to aid optimisation of medicines and provide core pharmacy duties. You will have a substantial clinical case load.
The Solent role will be based in the clinical pharmacy team in Southampton.
As well as the above you will be expected to:
Improve patient health and care through the execution and delivery of safe efficient and effective pharmacy services
Promote high quality cost effective and safe prescribing
Support the development of future services
Build and develop relationships across the PCN and Solent Southampton to enhance awareness of pharmaceutical issues.
NOTE: The post holder will report to the Lead Network Clinical Pharmacist and this post (Band 8a Clinical pharmacist /independent prescriber) is being advertised now and will be recruited to provide cross-sector pharmaceutical expertise and support to Solent NHS Community & Mental Health Trust and Southampton City (North) Primary Care Network.
